- On 131125 0005 research indicates aerobic exercise (walking, jogging,
- biking, that raises heart rate for 20 to 30 minutes at a time) may be
- the most effective way to increase HDL levels in a way that appears to
- lower risk of heart disease by regressing atherosclerotic plaques in
- blood vessels. The duration of exercise, rather than intensity (i.e.,
- speed) is the more important factor in raising HDL choleserol.

- On 131125 0005 research indicates LDL cholesterol occurs with two (2)
- protein cores, apo B or apo A1 (see diagrams). Apo B molecules are
- very small and highly associated with causing vascular disease. Apo
- A1 molecules are large and fluffy and much less likely to cause
- problems. ref SDS 19 1V4J
- ..
- On 131125 0005 research indicates small, dense LDL may increase risk
- for any given level of LDL. Studies suggest having small, dense
- particles may have up to 300 percent greater risk than having large
- LDL particles. ref SDS 19 PC5N
- ..
- On 131125 0005 research indicates Small, dense LDL correlates with
- high triglycerides (TG) > 140 and low HDL < 40. ref SDS 19 PD5I
- Triglycerides are lowered by exercise and weight loss, which also
- converts small, dense LDL particles into large, fluffy particles.
- ref SDS 19 PD69 Low TG < 70 indicates large, fluffy LDL particle size
- that cannot penetrate endothelium lining of blood vessels to cause
- atherosclerotic plaques. ref SDS 19 PD79
- ..
- On 131125 0005 research indicates statin drugs do not change LDL
- particle size appreciably. TG between 70 and 140 for patients with
- CAD consideration may be given to measuring LDL particle size.
- ref SDS 19 PD85

- On 131125 0005 research indicates consistent exercise lowers
- triglycerides, which are simply "fats" that circulate in the blood
- bound to particles called lipoproteins that also carry cholesterol.
- As they circulate, tissues pick up triglycerides to use for energy or
- store them to use as fuel later. Aerobic exercise for at least twenty
- minutes per session, preferably thirty or more minutes. For the first
- minutes of exercise, your body burns mostly glycogen, but if you
- exercise longer, your body switches to burning mostly triglycerides as

- ref SDS 2 MY4N Treatment to resolve achalasia was delayed 3 - 4
- years, which caused upper esophagus to expand (bulge), possibly

- Purpose of meeting today is to order standard of care testing for
- response to treatment of atherosclerosis that supports reducing
- treatment in order to reduce debilitating side effects. Current
- strategy is to test with coronary CT angiography (CCTA) and include

- meeting on 150213, with Doctor Rao, following patient cardiology care
- with Atorvastatin 10 mg and Ezetimibe 10 mg, the doctor noted that
- raising HDL 130% from 30 on 091022, when patient suffered severe
- atherosclerosis with CVD that required CABG x4, to reach HDL 70,
- reflects hiking 10,000 miles the past 4 years. ref SDS 33 ZM3I

- Doctor Simpson commented that years ago drugs were developed which
- reached HDL 90; however, this did not achieve regression of
- atherosclerotic plaques. This aligns with research finding that
- cardiac risk increased using drugs to increase HDL, reported on 131125
- 0005. ref SDS 19 KG9F
-
- [On 151204 1400 telecon with Doctor Vernaccia noted success

- He is doing extremely well w/o any CV sxs, esp no angina.
- Lipid panel is very good, and HDL is high, which he attributes
- to his vigorous exercise regimine. However, he feels that
- statin rx is causing xs urination and dry/cracked feet, and he
- would like to continue not taking it (he is not taking a statin
- now). He feels that a good CTA will support that, whereas a
- bad CTA would require further work.
- ..
- 9. PLAN:
-
- We had a very long discussion of the pros and cons. In my
- opinion, a "good" CTA would not rule out unstable and/or
- non-calcified plaque that might be prone to rupture and MI, and
- might be stabilized by statin rx. On the other hand, a "bad"
- CTA might present a quandry of what to do in a pt who is doing
- so well clinically. I also said that I don't think CTA is
- sufficiently sensitive to follow CAD, esp over short times, not
- to mention the xs radiation exposure would make this not
- indicated. Guidelines do not recommend this application.
- ..
- Nevertheless, he is very intent on getting this test, and I
- have ordered it. We will reassess after. Obviously we can
- adjust statin rx, although I doubt that statin is responsible
- for urinary frequency and/or dry feet.

- I agree with above. CTA is not a sensitive test to follow
- coronary lesions. Patient is doing very well but with his
- prior hx--indication for statin therapy is strong--- need to
- find a statin and dose of statin that he can tolerate. Patient
- has stopped me in hallway and I had similar discussion with him
- that CTA is not recommended for this purpose.
