041510 -        getting a CT scan that includes vital organs in the neck,
041511 -        chest, abdomen and pelvis, as we have been doing the past 8
041512 -        years.  You demurred seeming to indicate that nothing in a CT
041513 -        test would influence treatment, and so only the head was
041514 -        scanned yesterday to evaluate recent headaches [reported on

041518 -   12.  Tomorrow, I want to consider getting a full CT scan for best
041519 -        evidence on cancer spreading to internal organs and bones.  A
041520 -        big lump has developed the past few weeks at the left collar
041521 -        bone near my throat.  Bones seem to ache in my chest where
041522 -        radiation was performed in February and March 2009.  The past
041523 -        number of weeks I have felt bad inside my chest.  I forgot to
041524 -        mention this during the meeting last week, because there are so
041525 -        many things to remember [see report on 100112 0830. ref SDS 64
041526 -        448F].  Oxychodone has saved the day with welcome relief.  Some
041527 -        days I feel like my whole body is failing.  As noted, today I

041532 -   13.  Obviously, evidence of cancer spreading impacts Hospice
041533 -        considerations, listed in your medical chart.  If a CT test
041534 -        shows I am a "lost cause," I would prefer to spend my remaining
041535 -        time without the constant burden of medical logistics.  Can a
041536 -        CT scan provide a better picture on how much effort to invest
041537 -        on further treatment? [...requested during meeting at Kaiser on
041538 -        100112, and not ordered by Doctor Johnson on the grounds that
041539 -        CT testing would not influence treatment planning. ref SDS 64

041543 -   14.  During an earlier meeting at Kaiser last year, when I was
041544 -        hospitalized for painful breathing, you noted that even though
041545 -        the past 20 or so CT/PET/MRI tests over the past 8 years have
041546 -        never found evidence of cancer in my internal organs, there is
041547 -        likely cancer at very low levels, which is resisted by synergy
041548 -        between treatments and my immune system, strengthened with
041549 -        vigorous exercise.  This combination was very effective until
041550 -        July, when UCSF changed the prescription from "targeted" to
041551 -        general chemotherapy, first with Gemzar and now Navelbnine.
041552 -        While I got through the 8th relapse of cancer under this
041553 -        regimen, former synergy between treatment and exercise was
041554 -        reduced to zero.  Therefore, I am great risk until the team
041555 -        prescribes targeted treatment so that synergy is restored
041556 -        between treatment and exercise, as you explained during the
041557 -        meeting on 030606.

041560 -   15.  My last CT test on 090919 ordered in the ER to assess painful
041561 -        breathing found no evidence of cancer, not even in lymph nodes,
041562 -        including the neck, which have been early warnings in the past.
041563 -        Therefore, if a new CT test shows my cancer has not spread
041564 -        internally, e.g., to bones, lungs, liver, pancreas, etc., and
041565 -        if we can find a "targeted" treatment that does not have
041566 -        debilitating side effects, then I am prepared to continue work
041567 -        treating my "stage V" cancer as a "chronic disease," as you

041571 -   16.  Good news -- the lab last week, and again yesterday the lab on
041572 -        100119 show rising blood counts, signaling evident strength of
041573 -        my immune system, with ANC 5100 (NEUTS 5.1 at UCSF), just about
041574 -        the highest  I have ever had 8 years.  Elevated WBC 7.0 may
041575 -        reflect the immune system is reacting to shingles infection,
041576 -        but now my red blood cell counts have finally increased to
041577 -        within specified targets, after 4 - 5 months of low labs.

041581 -        declined after rising 2 weeks earlier to 248.  Improvement in

041636 -        occur in the next few weeks, based on patient history with
041637 -        sunitinib and Gemzar, particularly since we were forced to
041638 -        pause treatment due to shingles.  Investing time to be prepared
041639 -        for the 10th relapse of cancer, reflects your many warnings
041640 -        that because Kaiser and UCSF have been successful treating my
041641 -        stage IV cancer for 5 years, my cancer has moved to a much more
041642 -        aggressive "stage V" condition that requires starting the next
041643 -        treatment quickly following progression.  Since this is a new
041644 -        condition, there are few treatments available, opening an
041645 -        opportunity to innovate both treatment and s

041648 -   23.  In your review of PARP treatments last week, there was
041649 -        discussion that Kaiser typically does not support phase 1
041650 -        clinical trials, because this conflicts with evidenced-based
041651 -        medicine practiced at Kaiser.

041654 -   24.  In this case, Kaiser should make a reasonable effort to recover
041655 -        from UCSF medical mistakes failing to treat cancer in my
041656 -        breastbone area, which caused cancer to spread, and severely
041657 -        impacted my breathing, both beginning on 090401.  At that time,
041658 -        on 090401 I requested treatment.  UCSF delayed treatment until
041659 -        the "wound" in my breastbone healed, mistakenly believing this
041660 -        was reaction to radiation.  On 090609 Doctor Hsu in the UCSF
041661 -        Radiology Oncology Department discovered the "wound" is
041662 -        actually cancer that was untreated for nearly 6 months, causing
041663 -        cancer to spread.   UCSF took immediate control of the case and
041664 -        within 7 days on 090616, started me on MPA prescribed by Doctor
041665 -        Hsu to recover from radiation mistakes.

041668 -   25.  Doctor Rugo advised on 090616 that "everybody at Kaiser and
041669 -        UCSF is on board" for treating my cancer as a chronic disease
041670 -        targeted to my patient profile.  The doctor further indicated
041671 -        that after MPA, UCSF would treat me with PARP, as the newest
