-
- The doctor was able to remove all of the cancer in the left breast,
- indicating a successful surgery. Some lymph nodes under the left arm
- were identified as infected and were removed. The lymph node material
- appeared cancerous, but will be analysed by a lab. ref SDS 0 Q78M The
- surgeon will call Millie next week with results from the lab, and will
- meet with Millie on 020321 to discuss further treatment under the care
- of an oncologist. At this time there is no further indication of
- cancer; however, more testing will be recommended to verify the
- condition. It appears likely that radiation and some chemo therapy
- will be needed. ref SDS 0 Q78M Back home after surgery, Millie had a
- good laugh getting a report on her latest mammogram that said no
- cancer was found. ref SDS 0 YV3O Millie is grateful for everyone's
- support. Visits by Ron Jr., and Steve, and the flowers, calls and
- letters from family and friends lifted her spirits. ref SDS 0 H66F

- We arrived at the parking lot at the Kaiser hospital in Walnut Creek
- about 0910. There was plenty of parking.
- ..
- There were quite a few people in the waiting room for Nuclear Medicine
- when we arrived at 0915. However, promptly at 0930 as scheduled,
- Millie was called to get the injection, as explained by the surgeon

- ..
- We returned to Kaiser and went to Admitting for Surgery on the 2nd
- floor of the hospital.
- ..
- Had some difficulty parking because the lot was very crowded, with
- many cars waiting to find a stall. There was valet parking on the
- roof, but this required leaving the car keys with an attendant. We
- decided against this and eventually found a parking space.
- ..
- Millie signed in at the surgery admitting desk on the 2nd floor of the
- hospital, which is easily accessed from the main entrance to the
- hospital.
- ..
- A clerical nurse took Millie into a room to fill out more documents
- for surgery. The nurse asked questions and entered information into a
- computer.
- ..
- Pam arrived a few minutes later.
- ..
- The nurse used Millie's Kaiser card and driver's license to prepare an
- identification band for the patient's wrist. Millie then gave me her
- Kaiser card and driver's license to hold until after the surgery.
- ..
- After the documents were completed for admission to the surgery
- department, Millie was taken to a pre-op area.
- ..
- In leaving the Admissions room, Pam noticed the patient identification
- band was still on the desk. Pam had the presence of mind to pick it
- up and ask if this should be placed on her mom's wrist? The clerk
- thanked Pam for reminding about this. She took the ID band and
- fastened it to Millie's wrist, and advised that she is filling in
- today during lunch, because the Admitting Department is short handed.
- Admitting is not her normal job.
- ..
- Pre-op is just across the hall from Admitting. This is a very large
- room with perhaps 20 or 30 cubicles separated by curtains on a rail in
- the ceiling. The room seemed nearly full and there were 4 - 6 nurses
- and many aids and orderlies. A common workstation was in the
- center with room for each nurse to have a separate computer and a
- place to prepare documents
- ..
- Millie changed into a hospital patient's gown, and put her clothing in
- a plastic bag provided by the hospital with her name on the front.
- This bag can be stored in a closet until after the surgery, but we
- elected to hold them for Millie to reduce the chance of mix-ups.
- ..
- A male nurse took vital signs. Blood pressure was above 170. Millie
- said this morning her blood pressure was about 140. The nurse was
- reassuring in commenting that typically a surgery patient will have
- elevated vital signs from anticipating the seriousness of surgery. The
- pulse was about 55 showing a strong heart. More paper work was
- prepared using paper forms. Kaiser has a lot of computers and a lot
- of paper.
- ..
- Another nurse connected an IV into Millie's right arm.
- ..
- The nurse asked Millie where the surgery is to be performed. Millie
- confirmed the operation will be on the left breast. The nurse used a
- big felt marking pen to mark Millie's chest showing where the surgery
- is to occur. She said this helps avoid mistakes in the operating
- room.
- ..
- The nurse said the surgery will take an hour. After surgery, Millie
- will be taken to a recovery room. She will be in recovery for about
- an hour to awaken from the anesthetic. She will then be moved back to
- the area where are now for post-op support and processing for
- discharge to go home.

- surgery which had been scheduled yesterday for 1330. ref SDS 7 V48L
- ..
- When we got home about 1800 there was a message on the phone at
- 1000 asking that Millie come in early, because there was a surgery
- cancellation which enabled moving Millie up from 1330 to 1230.
- ..
- We heard the nurse saying on the telephone that surgery was ready for
- Millie now.
- ..
- The nurse came over and said Millie's surgery has been moved up.
- ..
- Another nurse showed up to move Millie to the operating room.
- ..
- A woman who had been in the cubicle next to Millie, said her surgery
- had been cancelled. She was dressed, and asked if Millie would like a
- prayer. Millie said yes, and so the woman said a prayer holding
- Millie's had. Millie held Pam's hand and the woman included Pam
- in a prayer.
- ..
- The nurse then pushed Millie's gurney out of the pre-op room and we
- followed down the hall to the entrance to the surgery area. The nurse
- directed Pam and I to the waiting room. We kissed Millie and wished
- her good luck.
- ..
- I left the waiting room briefly, and while walking down the hall the
- woman who said a prayer for Millie, saw me and commented further about
- having good feelings for Millie's recovery, from a religious
- perspective. She later was in a wheel chair for some reason that was
- not clear, since when we saw her in the pre-op room she seemed to be
- walking without any difficulty. A man was pushing the woman so she
- could speak to others in the waiting room and eventually the man
- pushed her over and she spoke to Pam and I further about her feelings
- arising from Millie's cancer.

- ..
- Surgery was successful. The doctor observed a small cancerous mass

- The pathology report prepared on the material removed today will be

- from observations of the removed lymph nodes, then prudence would
- require further diagnostics to look more carefully for other cancer.
- She seemed to indicate that a minimum regimen of radiation and chemo

- Breast cancer tests "flawed"
- ..
- ...saying in part that when women have a breast cancer removed,
- pathologists check their lymph nodes - a central part of the
- immune system found in the armpit - to see if any of the tumour
- has got into the blood stream.
- ..
- If it has, the cancer may recur or spread to another part of the
- body, so doctors use radiation or chemotherapy to destroy
- remaining cancer cells.
- ..
- The doctor further seemed to indicate that there are several levels of
- chemo therapy treatments. If further testing does not reveal more
- cancer, then a preventive maintenance, minimum chemo therapy program
- might be prescribed. If further diagnostics reveal cancer beginning
- to form elsewhere, then a medium program might be tried. If a serious
- problem exists, then aggressive treatment would be necessary.
-
- The same article further says in part...
- ..
- But the study, published in The Lancet medical journal, found that
- current tests missed many hidden tumours and their use was "no
- longer clinically tenable".

- The new method stains the tissue with a chemical that triggers an
- immune-system response to highlight anything that should not be
- there.
- ..
- Since at this time there is no indication of other cancer the
- implication is that a minimum preventive maintenance treatment may
- result. But we don't really know, and will not know until more
- diagnostics are performed.
- ..
- The surgeon will call Millie next week to discuss findings of the
- pathology report. A post-surgery follow up meeting with Millie is
- sceduled for 020321 1445 to discuss next steps. After that meeting,
- Millie will begin working with an oncologist to develop a diagonstic
- and treatment program.
- ..
- The doctor advised that the ace tube-top-like bandage from the surgery
- can be removed in the morning, and Millie can shower in 2 days. The
- underlying gauze dressing on the surgery incision should remain in
- place until it falls off by itself after about a week of showering.

- heard anything. This was a little troubling since the surgeon had

- ..
- Pam noticed a Reader's Digest among the magazines available in the
- waiting room, and drew attention to an article about mistakes in
- medical practice. We recalled the report on 990912 about the high
- cost of medical mistakes, ref SDS 1 0001, which led a few months later
- on 991207 to a national initiative on helping doctors, nurses and
- administrators strengthen medical practice. ref SDS 2 0001 Pam
- recalled a recent inquiry in the Kaiser Papers that raised concern
- about improvement needed at Kaiser. ref SDS 4 0001 Today, Pam's
- alertness helped avoid adding to the statistics, as we seemed to get
- through with a minimum of snafus.
- ..
- About 15 minutes later, an attendant said Millie has been moved to
- post-op and can receive visitors.
- ..
- Millie looked well under the circumstances. She seemed understandably
- tired from her ordeal. Her voice was slow and tired. The equipment
- was still connected (e.g., IV, breathing support). She was thirsty,
- so we got her some water. Made 3 or 4 trips for this.
- ..
- The nurse, Claire Conger, gave Millie some crackers to eat as a test
- to see how she is functioning in advance of release to go home.
- ..
- Earlier the post-op room had been almost empty, but now it was filling
- up again, and there seemed to be only one or maybe two nurses
- attending all of the patients. This meant that getting released was
- extended by about 30 to 40 minutes.
- ..
- Millie commented that there was some dissension among the nurses in
- the recovery room. She recalled hearing a supervisor say something
- about "writing up" one of the nurses for infractions in performing
- various duties. She was surprised to hear nurses arguing in the
- presence of patients. This issue may have contributed to the extra
- two hours in recovery, since obvsiously Millie was awake in Recovery,
- and then eventually went back to sleep. Pam and I did not observe any
- evident dissention among staff. Everyone seemed to be doing their
- jobs in a professinoal, efficient and congenial manner. In the
- surgery area where we went to ask about Millie there were 5 or 6
- doctors and nurses standing around laughing about something, so we did
- not observe the dissention that Millie recalled.
- ..
- After 30 - 40 minutes, the nurse in post-op began processing Millie
- for release. Pam helped Millie dress.
- ..
- Nurse Claire walked me over to a window and showed where to park the
- car in front of the hospital for emergency patient pickup. Millie has
- to be taken out of the facility in a wheelchair.

- released to walk out of the hospital. ref SDS 18 NE4T
- ..
- I went and got the car on the 5th floor of the parking garage, and
- drove to the emergency patient parking in front of the hospital on
- South Main. All of the parking spaces were taken. Parked along a
- curb that was not marked for parking, but other cars were there, so
- hoped this would be okay.
- ..
- Back in post-op, the nurse was still attending other patients.
- ..
- The nurse seemed really busy handling so many matters, i.e., new
- patients coming in and many to process for release, so we were patient
- and did not press to expedite Millie's release.
- ..
- Claire finally got to Millie and read through instructions on a form
- that included a restricted diet this evening. This conflicted with
- understandings from the surgeon, however, we did not review the
- matter.

- ..
- Millie was fitted with a sling which seemed to conflict with doctor's
- instructions that personal physical therapy should commence as soon as
- possible to minimize the formation of adhesions from the surgery under
- the arm. The nurse said the sling is only intended for today and
- possibly tomorrow. She noted instructions for PT to commence early.
- ..
- Nurse Clair wrote her name and phone number on the release forms that
- Pam signed for Millie, so if we got a ticket for parking in the wrong
- place downstairs, she would take care of it. She said this happens
- sometimes.
- ..
- We then had to round up an orderly with a wheelchair. Several
- patients were taken down, then it was Millie's turn.
- ..
- In leaving, we complimented nurse Clair for doing a good job handling
- so many patients without any help. She was comptent and attentive in
- performing her duties performed under a lot of stress from patients
- and family members anxious to be released, and others were being
- admitted. It looked like a lot for one person to handle. We were
