0507 - Summary/Objective
050801 - Follow up ref SDS 7 0000, ref SDS 5 0000.
050803 - The doctor was able to remove all of the cancer in the left breast,
050804 - indicating a successful surgery. Some lymph nodes under the left arm
050805 - were identified as infected and were removed. The lymph node material
050806 - appeared cancerous, but will be analysed by a lab. ref SDS 0 Q78M The
050807 - surgeon will call Millie next week with results from the lab, and will
050808 - meet with Millie on 020321 to discuss further treatment under the care
050809 - of an oncologist. At this time there is no further indication of
050810 - cancer; however, more testing will be recommended to verify the
050811 - condition. It appears likely that radiation and some chemo therapy
050812 - will be needed. ref SDS 0 Q78M Back home after surgery, Millie had a
050813 - good laugh getting a report on her latest mammogram that said no
050814 - cancer was found. ref SDS 0 YV3O Millie is grateful for everyone's
050815 - support. Visits by Ron Jr., and Steve, and the flowers, calls and
050816 - letters from family and friends lifted her spirits. ref SDS 0 H66F
050817 - All in all, Millie said today was very "productive." ref SDS 0 K83O

051203 - We arrived at the parking lot at the Kaiser hospital in Walnut Creek
051204 - about 0910. There was plenty of parking.
051206 - ..
051207 - There were quite a few people in the waiting room for Nuclear Medicine
051208 - when we arrived at 0915. However, promptly at 0930 as scheduled,
051209 - Millie was called to get the injection, as explained by the surgeon

Doctor's Comments Following Surgery
070501 - ..
070502 - Admitting for Surgery
0707 - 1110
070802 - ..
070803 - We returned to Kaiser and went to Admitting for Surgery on the 2nd
070804 - floor of the hospital.
070806 - ..
070807 - Had some difficulty parking because the lot was very crowded, with
070808 - many cars waiting to find a stall. There was valet parking on the
070809 - roof, but this required leaving the car keys with an attendant. We
070810 - decided against this and eventually found a parking space.
070812 - ..
070813 - Millie signed in at the surgery admitting desk on the 2nd floor of the
070814 - hospital, which is easily accessed from the main entrance to the
070815 - hospital.
070817 - ..
070818 - A clerical nurse took Millie into a room to fill out more documents
070819 - for surgery. The nurse asked questions and entered information into a
070820 - computer.
070822 - ..
070823 - Pam arrived a few minutes later.
070825 - ..
070826 - The nurse used Millie's Kaiser card and driver's license to prepare an
070827 - identification band for the patient's wrist. Millie then gave me her
070828 - Kaiser card and driver's license to hold until after the surgery.
070830 - ..
070831 - After the documents were completed for admission to the surgery
070832 - department, Millie was taken to a pre-op area.
070834 - ..
070835 - In leaving the Admissions room, Pam noticed the patient identification
070836 - band was still on the desk. Pam had the presence of mind to pick it
070837 - up and ask if this should be placed on her mom's wrist? The clerk
070838 - thanked Pam for reminding about this. She took the ID band and
070839 - fastened it to Millie's wrist, and advised that she is filling in
070840 - today during lunch, because the Admitting Department is short handed.
070841 - Admitting is not her normal job.
070843 - ..
070844 - Pre-op is just across the hall from Admitting. This is a very large
070845 - room with perhaps 20 or 30 cubicles separated by curtains on a rail in
070846 - the ceiling. The room seemed nearly full and there were 4 - 6 nurses
070847 - and many aids and orderlies. A common workstation was in the
070848 - center with room for each nurse to have a separate computer and a
070849 - place to prepare documents
070851 - ..
070852 - Millie changed into a hospital patient's gown, and put her clothing in
070853 - a plastic bag provided by the hospital with her name on the front.
070854 - This bag can be stored in a closet until after the surgery, but we
070855 - elected to hold them for Millie to reduce the chance of mix-ups.
070857 - ..
070858 - A male nurse took vital signs. Blood pressure was above 170. Millie
070859 - said this morning her blood pressure was about 140. The nurse was
070860 - reassuring in commenting that typically a surgery patient will have
070861 - elevated vital signs from anticipating the seriousness of surgery. The
070862 - pulse was about 55 showing a strong heart. More paper work was
070863 - prepared using paper forms. Kaiser has a lot of computers and a lot
070864 - of paper.
070866 - ..
070867 - Another nurse connected an IV into Millie's right arm.
070869 - ..
070870 - The nurse asked Millie where the surgery is to be performed. Millie
070871 - confirmed the operation will be on the left breast. The nurse used a
070872 - big felt marking pen to mark Millie's chest showing where the surgery
070873 - is to occur. She said this helps avoid mistakes in the operating
070874 - room.
070876 - ..
070877 - The nurse said the surgery will take an hour. After surgery, Millie
070878 - will be taken to a recovery room. She will be in recovery for about
070879 - an hour to awaken from the anesthetic. She will then be moved back to
070880 - the area where are now for post-op support and processing for
070881 - discharge to go home.

080401 - ..
080402 - Surgery Advanced from 1330 to 1230
0807 - 1230
0809 - ..
080901 - After the nurse completed setting up the IV, she went to her desk a
080902 - few feet away.
080904 - ..
080905 - Pam and I visited with Millie expecting a wait of about 60 minutes for
080906 - surgery which had been scheduled yesterday for 1330. ref SDS 7 V48L
080908 - ..
080909 - When we got home about 1800 there was a message on the phone at
080910 - 1000 asking that Millie come in early, because there was a surgery
080911 - cancellation which enabled moving Millie up from 1330 to 1230.
080913 - ..
080914 - We heard the nurse saying on the telephone that surgery was ready for
080915 - Millie now.
080917 - ..
080918 - The nurse came over and said Millie's surgery has been moved up.
080920 - ..
080921 - Another nurse showed up to move Millie to the operating room.
080923 - ..
080924 - A woman who had been in the cubicle next to Millie, said her surgery
080925 - had been cancelled. She was dressed, and asked if Millie would like a
080926 - prayer. Millie said yes, and so the woman said a prayer holding
080927 - Millie's had. Millie held Pam's hand and the woman included Pam
080928 - in a prayer.
080930 - ..
080931 - The nurse then pushed Millie's gurney out of the pre-op room and we
080932 - followed down the hall to the entrance to the surgery area. The nurse
080933 - directed Pam and I to the waiting room. We kissed Millie and wished
080934 - her good luck.
080936 - ..
080937 - I left the waiting room briefly, and while walking down the hall the
080938 - woman who said a prayer for Millie, saw me and commented further about
080939 - having good feelings for Millie's recovery, from a religious
080940 - perspective. She later was in a wheel chair for some reason that was
080941 - not clear, since when we saw her in the pre-op room she seemed to be
080942 - walking without any difficulty. A man was pushing the woman so she
080943 - could speak to others in the waiting room and eventually the man
080944 - pushed her over and she spoke to Pam and I further about her feelings
080945 - arising from Millie's cancer.

110802 - Surgery Completed, Discussion with Doctor on Results
110804 - The surgeon visited with Pam and I about results of the operation.
110806 - ..
110807 - The doctor described the surgery as a left breast sentinel node
110808 - procedure along with a left axillary disection and left breast
110809 - lumpectomy.
110811 - ..
110812 - Surgery was successful. The doctor observed a small cancerous mass
110813 - (tumor) about the size of a dime in Millie's breast. The surrounding
110814 - area about twice that size was removed as a precaution to avoid
110815 - regeneration of cancer by creating clear margins between the tumor and
110816 - uninfected tissue.
110818 - [On 020318 the surgeon called and reported preliminary results

110881 - far do not indicate there is more cancer in the body.

110894 - The pathology report prepared on the material removed today will be
110895 - available in a few days. If the material is cancerous as she suspects
110896 - from observations of the removed lymph nodes, then prudence would
110897 - require further diagnostics to look more carefully for other cancer.
110898 - She seemed to indicate that a minimum regimen of radiation and chemo
110899 - therapy may be recommended by the oncologist in the next phase of
110900 - Millie's recovery from cancer.

110903 - Research on the Internet seems to align with the doctor's

110906 - http://news.bbc.co.uk/hi/english/health/newsid_443000/443256.stm
110908 - ..
110909 - Reports an article...
110911 - Breast cancer tests "flawed"
110913 - ..
110914 - ...saying in part that when women have a breast cancer removed,
110915 - pathologists check their lymph nodes - a central part of the
110916 - immune system found in the armpit - to see if any of the tumour
110917 - has got into the blood stream.
110919 - ..
110920 - If it has, the cancer may recur or spread to another part of the
110921 - body, so doctors use radiation or chemotherapy to destroy
110922 - remaining cancer cells.
110924 - ..
110925 - The doctor further seemed to indicate that there are several levels of
110926 - chemo therapy treatments. If further testing does not reveal more
110927 - cancer, then a preventive maintenance, minimum chemo therapy program
110928 - might be prescribed. If further diagnostics reveal cancer beginning
110929 - to form elsewhere, then a medium program might be tried. If a serious
110930 - problem exists, then aggressive treatment would be necessary.
110932 - The same article further says in part...
110934 - ..
110935 - But the study, published in The Lancet medical journal, found that
110936 - current tests missed many hidden tumours and their use was "no
110937 - longer clinically tenable".
110939 - ..
110940 - The new method stains the tissue with a chemical that triggers an
110941 - immune-system response to highlight anything that should not be
110942 - there.
110944 - ..
110945 - Since at this time there is no indication of other cancer the
110946 - implication is that a minimum preventive maintenance treatment may
110947 - result. But we don't really know, and will not know until more
110948 - diagnostics are performed.
110950 - ..
110951 - The surgeon will call Millie next week to discuss findings of the
110952 - pathology report. A post-surgery follow up meeting with Millie is
110953 - sceduled for 020321 1445 to discuss next steps. After that meeting,
110954 - Millie will begin working with an oncologist to develop a diagonstic
110955 - and treatment program.
110957 - ..
110958 - The doctor advised that the ace tube-top-like bandage from the surgery
110959 - can be removed in the morning, and Millie can shower in 2 days. The
110960 - underlying gauze dressing on the surgery incision should remain in
110961 - place until it falls off by itself after about a week of showering.
110963 - ..
110964 - She said there are no restrictions on eating.
110966 - ..
110967 - Personal physical therapy exercises were explained yesterday,
110968 - ref SDS 7 RK7O, to recover from adhesions that will result from
110969 - incisions under the arm,
110971 - ..
110972 - The doctor said that Millie will be in recovery for about an hour
110973 - until 1445, then she will be moved to post-op which is the same room
110974 - where were with Millie for pre-op. Millie will be processed for
110975 - release from post-op in about another hour.

120905 - Millie and I had stopped there earlier this morning to get a watch
120906 - fixed, but the shop did not open until 1130.
120907 - ..
120908 - We got the watched fixed.
120910 - ..
120911 - Pam used her cell phone to try calling Marilyn, Millie's childhood
120912 - friend from Chicago.
120914 - ..
120915 - The line was busy for several hours.
120917 - ..
120918 - She called Bill, maybe Steve and Ron Jr., since they all have cell
120919 - phones.
120921 - ..
120922 - Interestingly, the admitting nurse had a place in the online form to
120923 - get a cell phone number for Pam as an additional contact method,
120924 - indicating the cell phone is becoming a universal presence.
120926 - ..
120927 - Pam may have finally gotten through to Marilyn and related results of
120928 - surgery. Pam mentioned that Marilyn is available to visit and care
120929 - for Millie during recovery from surgery and the coming treatments.
120930 - Marilyn's sister Bernie recently passed away after a long battle with
120931 - cancer, and so Marilyn's experience caring for her sister is a great
120932 - aid and comfort in helping Millie with what lies ahead.

121301 - By this time Millie should have been moved from recovery to the

121304 - ..
121305 - Since we had not heard anything, Pam and I walked over to post-op.
121307 - ..
121308 - The nurse said they have not have heard anything. We expected Millie
121309 - any moment, so we went to the admitting area just outside the post-op
121310 - room, rather than return to the waiting room for surgery, which is
121311 - across the hall. The nurse said she would come and get us, as soon as
121312 - Millie is brought in.
121314 - ..
121315 - An hour later, after checking several more times we still had not
121316 - heard anything. This was a little troubling since the surgeon had
121317 - indicated Millie would be returned to post-op by about 1445.
121318 - ref SDS 0 H56N
121320 - ..
121321 - We walked to the surgery area. A doctor or nurse offered to assist
121322 - us, and we asked if Millie is still in recovery? The woman left and
121323 - returned about 5 minutes later, and said Millie is still sleeping in
121324 - the recovery room. They will wake her in about 20 minutes and take
121325 - her to post-op.

130903 - ..
130904 - Pam noticed a Reader's Digest among the magazines available in the
130905 - waiting room, and drew attention to an article about mistakes in
130906 - medical practice. We recalled the report on 990912 about the high
130907 - cost of medical mistakes, ref SDS 1 0001, which led a few months later
130908 - on 991207 to a national initiative on helping doctors, nurses and
130909 - administrators strengthen medical practice. ref SDS 2 0001 Pam
130910 - recalled a recent inquiry in the Kaiser Papers that raised concern
130911 - about improvement needed at Kaiser. ref SDS 4 0001 Today, Pam's
130912 - alertness helped avoid adding to the statistics, as we seemed to get
130913 - through with a minimum of snafus.
130915 - ..
130916 - About 15 minutes later, an attendant said Millie has been moved to
130917 - post-op and can receive visitors.
130919 - ..
130920 - Millie looked well under the circumstances. She seemed understandably
130921 - tired from her ordeal. Her voice was slow and tired. The equipment
130922 - was still connected (e.g., IV, breathing support). She was thirsty,
130923 - so we got her some water. Made 3 or 4 trips for this.
130925 - ..
130926 - The nurse, Claire Conger, gave Millie some crackers to eat as a test
130927 - to see how she is functioning in advance of release to go home.
130929 - ..
130930 - Earlier the post-op room had been almost empty, but now it was filling
130931 - up again, and there seemed to be only one or maybe two nurses
130932 - attending all of the patients. This meant that getting released was
130933 - extended by about 30 to 40 minutes.
130935 - ..
130936 - Millie commented that there was some dissension among the nurses in
130937 - the recovery room. She recalled hearing a supervisor say something
130938 - about "writing up" one of the nurses for infractions in performing
130939 - various duties. She was surprised to hear nurses arguing in the
130940 - presence of patients. This issue may have contributed to the extra
130941 - two hours in recovery, since obvsiously Millie was awake in Recovery,
130942 - and then eventually went back to sleep. Pam and I did not observe any
130943 - evident dissention among staff. Everyone seemed to be doing their
130944 - jobs in a professinoal, efficient and congenial manner. In the
130945 - surgery area where we went to ask about Millie there were 5 or 6
130946 - doctors and nurses standing around laughing about something, so we did
130947 - not observe the dissention that Millie recalled.
130949 - ..
130950 - After 30 - 40 minutes, the nurse in post-op began processing Millie
130951 - for release. Pam helped Millie dress.
130953 - ..
130954 - Nurse Claire walked me over to a window and showed where to park the
130955 - car in front of the hospital for emergency patient pickup. Millie has
130956 - to be taken out of the facility in a wheelchair.
130958 - [On 051022 Millie had a much more serious operation, and was
130959 - released to walk out of the hospital. ref SDS 18 NE4T
130961 - ..
130962 - I went and got the car on the 5th floor of the parking garage, and
130963 - drove to the emergency patient parking in front of the hospital on
130964 - South Main. All of the parking spaces were taken. Parked along a
130965 - curb that was not marked for parking, but other cars were there, so
130966 - hoped this would be okay.
130968 - ..
130969 - Back in post-op, the nurse was still attending other patients.
130971 - ..
130972 - The nurse seemed really busy handling so many matters, i.e., new
130973 - patients coming in and many to process for release, so we were patient
130974 - and did not press to expedite Millie's release.
130976 - ..
130977 - Claire finally got to Millie and read through instructions on a form
130978 - that included a restricted diet this evening. This conflicted with
130979 - understandings from the surgeon, however, we did not review the
130980 - matter.

130988 - ..
130989 - Millie was fitted with a sling which seemed to conflict with doctor's
130990 - instructions that personal physical therapy should commence as soon as
130991 - possible to minimize the formation of adhesions from the surgery under
130992 - the arm. The nurse said the sling is only intended for today and
130993 - possibly tomorrow. She noted instructions for PT to commence early.
130995 - ..
130996 - Nurse Clair wrote her name and phone number on the release forms that
130997 - Pam signed for Millie, so if we got a ticket for parking in the wrong
130998 - place downstairs, she would take care of it. She said this happens
130999 - sometimes.
131001 - ..
131002 - We then had to round up an orderly with a wheelchair. Several
131003 - patients were taken down, then it was Millie's turn.
131005 - ..
131006 - In leaving, we complimented nurse Clair for doing a good job handling
131007 - so many patients without any help. She was comptent and attentive in
131008 - performing her duties performed under a lot of stress from patients
131009 - and family members anxious to be released, and others were being
131010 - admitted. It looked like a lot for one person to handle. We were
131011 - grateful for her professionalism caring for Millie.
131013 - ..
131014 - Took about 15 minutes to drive home.
