330935 -    3.  My records show this will start the 16th overall cycle of
330936 -        cetuximab since 070201, and the 2nd cycle with Carboplatin
330937 -        added, beginning last month on 080306.  The 1st cycle with both
330938 -        cetuximab and Carboplatin had only 1 treatment with
330939 -        Carboplatin, rather than 3, because I asked to defer starting
