- Millie received a letter from NP dated 080225, asking about Millie's
- status on medical leave. Today, Millie submitted a letter explaining
- medical leave status continues due to relapse of cancer, and starting
- a new treatment, the 2nd arm of the cetuximab and carboplatin clinical
- trial...

- 1. Subject: Leave of Absence and Interactive Process
- Date: Tue, 01 Apr 2008 15:38:41 -0700
- ..
- 3. Thanks very much for your call yesterday afternoon, following
- up the letter on February 25th.
- ..
- 4. A few weeks ago I submitted documentation you requested to the
- doctor at UCSF. Doctor Rugo explained they have a department
- or group that does the processing. I am meeting with Doctor
- Rugo on Thursday to hopefully adjust my treatment for better
- results, and will ask about progress responding to NP's
- documentation.
- ..
- 5. I guess you heard that on February 19th the doctor determined I
- have suffered another relapse of IBC disease. This was a heart
- breaker because I had been doing well. UCSF scheduled
- increased treatment beginning on March 13th. However, there
- have been set backs starting the new regimen, which will be
- addressed on Thursday.
- ..
- 6. As a result, my status with NP remains unchanged. If the new
- treatment is effective, and side effects are not debilitating
- then prospects for returning to work seem good. We should know
- within a few cycles. I am sending a copy to Jackie, who
- coordinates case management for Doctor Rugo. She has been
- outstanding assisting me with a great many details, so please
- feel free to contact Jackie if you need more technical
- information on my medical condition.
- ..
- 7. Thanks very much for initiating an interactive process on my
- status, noted in your letter on February 25th. One of the big
- challenges I face is exposure to infectious disease.
- Chemotherapy reduces the immune system, so being around people,
- like at the office, but especially taking BART for several
- hours a day, increases my risks. At the same time, all of my
- doctors urge keeping involved, because activity strengthens the
- immune system.
- ..
- 8. So in a way working is good therapy provided we can moderate
- risks, and debilitating effects of chemo can be overcome.
- Since we are just beginning this treatment, we need a little
- experience to fully assess prospects.
- ..
- Millie's primary care physician at Kaiser reported on 030606 that
- cancer patients recover better by staying involved with work and
- routine activity at home, as much as possible. ref SDS 1 LZ3L On
- 041019 Doctor Guardino made a similar recommendation during a meeting
- at Stanford Cancer Center. ref SDS 2 NN6N
-
- [On 080725 Millie notifies NP experience on 2nd arm of
- clinical study demonstrates sufficient recovery from cancer
- relapse in February for Millie to return to work, as
- recommended by her doctor for increasing recovery.

- [On 080729 N&P requests clearance from physician stating
- limitations Millie may have on working. ref SDS 4 RG8P
- ..
- [On 080804 Millie notified the doctor at UCSF that NP
- requests physician release to work. ref SDS 5 GU4W
- ..
- [On 080807 UCSF responded and approved Millie for return to
- work. ref SDS 6 SX4O
- ..
- [On 080807 Millie thanks UCSF for timely response, and
- submitted to NP the UCSF letter with physician release for
- return to work from medical leave. ref SDS 7 AF5J
- ..
- [On 080808 Tim Blevins at NP requested signed copy of UCSF
- physician release from medical leave for Millie to return
- to work; and to delay starting work on 080811 so that NP
- has time to review recent changes to staffing. ref SDS 8

- Millie's letter to NP continues...
-
- 9. One form of accommodation, which you raise in your letter,
- would be to work from home, telecomuting, so to speak. I have
- used this type of support from time to time when things get
- backed up. My familiarity with N&P formats and requirements
- might make this a reasonable solution.
- ..
- 10. Thanks again to you and firm for consideration in helping me
- work through this transition.
- ..
- 11. I'll call tomorrow to explore further ideas.
