Dear Sheridan,
I have written and called several times, but have not heard from
you since your letter on Oct 7 saying you have not heard from me. On Oct
I wrote to you
explaining my medical records have been transferred to
UCSF in accordance with your instructions on Oct 6.

Today, I would like to fax the "Patient Information" forms filled out from the
download accessed in the attachment to your letter of Oct 7. UCSF's
instructions say to list on the fax cover sheet the date, time, and name of the
doctor. Can you get this to me, or should I reference your name in the cover
sheet for this information?

Yesterday, Rod called Kaiser and found that they messed up transferring the
medical records, so we have arranged to hand carry the records. I plan to
pick them up at the Business Office on Thursday when I have a treatment.
Your last letter on Oct 7 said a meeting could be this Friday, Oct 15, or
the following Monday. When time permits, please let me know the date, time
and the name of the doctor.
